Does Kinsuikan allow tattoos? Yes, covered
Kinsuikan draws its own spring under the island, a seawater-bearing shiozu held at 40.5 to 41.5°C so that guests can sit in it longer, which the inn says is the point of a salt spring.
The large bath is closed to tattooed guests unless the tattoo is covered. The inn’s own Q&A says bathing with a tattoo is refused, then adds that it is allowed if you hide it yourself with something like a tattoo seal, and that the inn keeps none to give you. Bring your own cover, in the size your tattoo needs. The bath page separately bars decorative tattoo stickers and body paint, so a painted-on design is not a way around the rule.

Visit Hiroshima Tourist Pass One, two or three days on Hiroshima’s streetcars, including the line out to Miyajimaguchi, and the ferry across to the island. Slower than the JR train, and it covers the boat. Sold on Klook.

Confirmed from official site (the inn’s own Q&A page). Its bath page carries a separate, blunter line refusing tattoos, tattoo stickers and body paint, so the two pages have to be read together: the Q&A is the one that sets out what a covered tattoo may do.

Best for: Bathing in Miyajima’s own seawater spring rather than a heated tub
Large bath 15:00 to 01:00 and 05:00 to 10:00; closed for cleaning 01:00–05:00 and 10:00–15:00. No day-use bathing at either this inn or its sister Hotel Miyajima Villa. The suites and special rooms with a semi-open-air tub are not on the spring: the inn labels that water 光明石 (artificial mineral) and says of one room type that it is heated water and not a hot spring. The cover-seal route is therefore the only way to the real spring water here.
