Does Kitakobushi Shiretoko Hotel & Resort allow tattoos? In a private bath
The hotel that sits closest to the drift ice in Utoro keeps no family bath at all, and instead puts the spring on the terraces of five room types, diluted and reheated but open to the Okhotsk sky.
The largest house in Utoro refuses the shared floor and then sells you a way around it. Its own FAQ answers the tattoo question with a request to refrain from the large baths, and a second answer says the hotel has neither a family bath nor a rental bath. What it does have is five room types with a hot-spring bath of their own, three of them with an open-air tub on the terrace, and the hotel states in the room descriptions that the water in those tubs is natural hot spring with water and heat added. That terrace, not the bathhouse, is the answer here.

The room types with their own hot-spring bath are オホーツク倶楽部サウナ付スパスイート, オホーツク倶楽部露天風呂付スイート, オホーツク倶楽部露天風呂付DXツイン, 温泉付モダンツイン and 温泉付ユニバーサルルーム. The hotel says of the terrace tubs 露天風呂は天然温泉（加水・加温あり）, so the water is spring water but diluted and reheated. Every room has a shower booth and a washlet; whether there is a tub depends on the room type. There is no family bath and no rental bath, so the only private water here is the room you book.
