Banshokaku Shikishima
萬象閣敷島
Does Banshokaku Shikishima allow tattoos? In a private bath
A hillside inn where the room, not the bathhouse, is the bathing space: oak and hinoki tubs, stone walls, a bed set beside the tree the room is named for. The smallest type is reserved for guests in their twenties and thirties.
Every room here has an open-air or semi-open-air bath of its own, from the compact 50 square metre type up to the 80 square metre suite, so a stay does not require the shared bath at all. The inn publishes a long FAQ that covers cancellation, wheelchairs and pets, and says nothing about tattoos.

What happens when you arrive
Skip the shared baths and reserve the private one the way this page describes. Once the door is closed, the bath is yours.
How we know
Confirmed from official site — bath structure, not a tattoo policy (the rooms page, where all five room types are named for their bath)

Best for: A wide open-air bath room built for two adults, with children under school age not accepted
Five room types: suites over 70 square meters with a lavish semi-open-air bath, a 70 square metre premium with a wide open-air bath room, two deluxe grades over 60 square meters (one looking onto an oak), and a 50 square metre type sold only to guests in their twenties and thirties. Children from birth to pre-school age are not accepted, and some rooms are adults only. Shared shuttle wagon from Ureshino-Onsen STN by prior arrangement. The second parking lot is shared with the day-use private baths next door, which are a separate business.
