Does Spa & Hotel Maihama Eurasia allow tattoos? Yes, covered
The spa allows tattoos only under up to four of its own seals, 10 cm × 15 cm (3.9 × 5.9 in) each. For anything larger, the balcony rooms have the spring water piped into their own tub.
The spa's own terms set the rule. Its seals are bought at the spa counter, seals you bring aren't accepted, and anything larger is refused in any circumstances. The balcony rooms' bathroom has a washing area with the spring water piped in.

The bath
The spa is the draw for most guests: the resort's only natural hot spring, with open-air baths, saunas and a rooftop terrace. Its terms are exact about tattoos: at most four of the counter's own 10 cm × 15 cm (3.9 × 5.9 in) seals, nothing you brought, and nothing larger under any circumstances. That makes the room matter. The Balcony and Deluxe Balcony rooms, 27 and 36 m², have a proper bathroom with a washing area and the spring water piped into the tub. A guest the spa won't admit still bathes in the onsen, alone, with the door shut.
The hotel
A spa hotel on the edge of the Maihama resort, with a free shuttle from Maihama Station and the Disney gates that also serves Urayasu and Shin-Urayasu. Rooms run from 26 m² standards to the balcony rooms facing the resort, with Japanese-style rooms and a high-floor premium grade above. The standard rooms have ordinary baths. It's the balcony grades that get the spring.

Buy the spa’s own seals at its counter, four at most, and cover the tattoo before you go in; seals you bring aren’t accepted. For anything larger, use the tub in a balcony room.
